New Strategy to prevent young people with diabetes dropping out of health care

The Youth Transiton Pack was successfully launched in Melbourne on 28 July 2010.

The purpose of the new Pack is to engage young people with diabetes and their parents/carers.  This will be done through sending annual birthday cards to the young person with diabetes from their twelfth to their twentieth birthdays accompanied by a letter detailing age-appropriate transition information for the parent.  It is hoped that this coordinated approach will encourage families to speak about transition from an early stage which will facilitate ongoing engagement in the healthcare system by young adults with diabetes. This initiative was developed by the National Diabetes Services Scheme (NDSS), administered by Diabetes Australia and funded by the Department of Health & Ageing.
