Change of Contact Details
It is important that Diabetes Australia has an up-to-date record of contact details for Registrants. If you change your address, phone number, or medical situation, you should complete a Change of Details Form and send it to Diabetes Australia, GPO Box 9824 in your capital city, or hand it to an NDSS Access Point (NDSS Outlet).

Misplaced, Lost or Damaged NDSS Card?
If you have misplaced your NDSS card or it is damaged, contact your state or territory Diabetes Australia office to organise a new one. You can phone your local Diabetes Australia office during business hours from anywhere in Australia on 1300 136 588 (local call cost). Back to top

NDSS Purchases for the Last Financial Year
There is a report you can get from your state or territory Diabetes Australia office, which details all of your purchases for the last financial year, for tax return purposes. Please call your local Diabetes Australia office on 1300 136 588. You will need to give them your NDSS Registration number so they can run the appropriate report. Back to top
