If you have been diagnosed with diabetes and would like to access the NDSS you must:
- complete an NDSS Registration Form (non gestational diabetes)
- complete an NDSS Registration Form (gestational diabetes)
- have it signed by your doctor or a Credentialled Diabetes Educator (CDE)
- mail it to Diabetes Australia, GPO Box 9824 in your Capital City, OR visit the Diabetes Australia office in your Capital City.

There is no charge to register with the NDSS, and the process only needs to be done once.

To register for the NDSS, you need to complete and submit an NDSS Registration Form. Section 3 of the Registration Form needs to be completed by your doctor or a Credentialed Diabetes Educator (CDE).

Shortly after you first register with the NDSS, you will receive a Registration Card and an Information & Starter Pack.
The pack aims to give new Registrants information about the NDSS, including the services and products available to Registrants, and provides general information about diabetes.
