The NDSS Order Form contains a list of all the products available under the NDSS and prices payable. Please read the following guidelines and conditions associated with ordering products.

Purchasing Guidelines

To purchase products through the NDSS you must be registered with the NDSS. You do not need to have a prescription from a doctor.

Additional Subsidies

Registrants who have one of the following cards are eligible for additional subsidies:
  • Health Care Card
  • Pensioner Concession Card
  • Safety Net Card
  • Veterans’ Affairs Card.

The NDSS Order Form specifies the price payable for each product based on the additional subsidy available.

No Exchanges

Diabetes Australia is not permitted to exchange NDSS products incorrectly ordered by Registrants. In addition, some products have a use-by date, so to avoid unnecessary wastage, you should not over-order these items.

Delivery of Product

All products sent by mail are free of any postage charges. Five working days should be allowed for delivery (10-14 days in remote areas).

Where to Get an Order Form

Download: NDSS Order Form. Alternatively, you can pick up order forms from any NDSS outlet. The Order Form is regularly updated. Make sure you have the latest version.

What to do With the Order Form

You should make sure you complete all the relevant sections of the Order Form. In particular, take care to provide the details of any concession cards, so you don’t miss out on any additional subsidies.

Once you have filled out the Order Form with the products you require, you can:

  • take it to your nearest NDSS outlet.
  • post it to Diabetes Australia, GPO Box 9824 in your Capital City.
