Annual cycle of care screening program

For people living with type 1 diabetes or type 2 diabetes

Diabetes changes over time, so it’s a good idea to keep your diabetes care on track at all times. Get help on how to do this with the annual cycle of care screening program.

Our trained health professionals perform a number of tests and measurements to identify any problems early on to lower your risk of diabetes-related complications.

These tests include:

  • HbA1C (three-month average of your blood glucose levels)
  • cholesterol
  • kidney health check
  • foot examination
  • blood pressure
  • BMI

During this workshop, you will have the chance to ask any questions you may have about your diabetes. You can also bring your blood glucose meter to be calibrated for accuracy.

Note: This screening is not a substitute for regular visits with your local diabetes health care team. If you are not sure if you should attend, talk to your doctor about what’s best for you.
