As an authorised health professional, sign up to the NDSS Health Professional Portal with your AHPRA or CDE registration number and you’ll be able to register the people in your care within minutes. The following health professionals are authorised to use the NDSS Health Professional Portal to certify NDSS forms: Aboriginal and Torres Strait Islander Health Practitioners Credentialled Diabetes Educator (CDE) Endocrinologist/Diabetologist General Practitioner Other registered medical practitioner who specialises in diabetes.
