Diabetes referral pathways for health professionals

Connecting your patients to the NDSS gives them free access to trusted information and education programs for every stage of their diabetes journey.

The NDSS supports people living with diabetes from diagnosis through everyday management, life changes, and healthy ageing, with free programs, resources, and subsidised products tailored to their needs. Whether your patient has just been diagnosed, needs culturally safe or translated support, or is navigating a new life stage, there is a pathway designed to help. Use the options below to quickly connect your patient with the most relevant NDSS service.

Just diagnosed

Just diagnosed with type 1 diabetes

Type 1 Diabetes and Me supports people newly diagnosed with type 1 diabetes with practical information on day-to-day management, emotional wellbeing, working with diabetes health professionals, and accessing NDSS support.

Just diagnosed with type 2 diabetes

Type 2 diabetes and me offers short online modules covering diabetes basics, healthy eating, physical activity, medication, and emotional wellbeing, helping newly diagnosed people build confidence in self-managing their diabetes.

Just diagnosed with gestational diabetes

This pathway helps people newly diagnosed with gestational diabetes understand what it means, how it’s managed during pregnancy, and where to find further NDSS information and support, including for the period after their baby is born.

Tailored support

Aboriginal and Torres Strait Islander peoples

This pathway connects Aboriginal and Torres Strait Islander peoples with culturally relevant NDSS resources and community-based support, including the Diabetes Yarning program, to help them manage diabetes with confidence.

Translated support

NDSS provides a range of diabetes information translated into more than 20 languages, helping people access trusted fact sheets, booklets, and videos in formats that suit their language needs.

Diabetes and disability

This pathway provides people living with diabetes and an intellectual disability with resources to help them learn about diabetes and prepare for health professional visits, as well as guidance on using their diabetes visit cards.

Mental health support

Looking after your mind is NDSS’s dedicated hub for the emotional and mental health impacts of living with diabetes, helping people recognise diabetes distress and burnout, plus find practical tools and support.

Life changes

Planning pregnancy

This pathway helps people with type 1 or type 2 diabetes plan and prepare for pregnancy, with an online checklist, health information, and guidance on sexual and reproductive health.

Starting school

Diabetes in Schools supports families, schools, and health professionals to work together so students with type 1 diabetes can manage their condition safely and fully participate at school.

Starting insulin for type 2 diabetes

This pathway helps people with type 2 diabetes understand what starting insulin involves, finding support, and accessing free diabetes programs available face-to-face or online.

Healthy ageing

This pathway provides information and support to help older people live well with diabetes, covering topics such as staying active, healthy eating, reducing the risk of complications, and accessing aged care support as their needs change.

Becoming independent

Diabetes Youth Zone gives young people with type 1 diabetes practical information and tools to build confidence, manage everyday life, school and study, and prepare for the transition to adult diabetes care.
