The Australian Government provides access to subsidised CGM for eligible people registered with the NDSS. CGM devices help people living with type 1 diabetes, or other rare conditions, to keep their blood glucose levels in their target range. This can help reduce the risk of severe low glucose events and diabetes-related complications. Who can access subsidised CGM products All people living with type 1 diabetes can apply for access to subsidised CGM products, including: children and young people aged under 21 years adults aged 21 years or older women who are actively planning pregnancy, pregnant, or immediately post-pregnancy Children and young people under 21 years with conditions similar to type 1 diabetes and who need insulin may also apply. How to access CGM products Step 1: Talk with an authorised diabetes health professional See an authorised diabetes health professional. They can check if you are eligible and help you choose a CGM product that suits your needs. Authorised health professionals include: endocrinologists (diabetes specialist doctors) credentialled diabetes educators physicians or paediatricians who specialise in diabetes. General practitioners and practice nurses are not authorised to certify CGM access forms. Credentialled diabetes educators are not authorised to certify CGM access forms for young people diagnosed with “other” eligible conditions. Step 2: Complete and submit a CGM access form Your authorised health professional may complete your access form online through the NDSS Health Professional Portal. Your access is usually confirmed within minutes once complete. You can also: download and complete the relevant CGM product access form ask your authorised health professional to certify the form submit the form by: email to [email protected] post to GPO Box 9824 in your capital city fax to 1300 536 953 Once your access is confirmed, we will send you an email or letter explaining how to access your products. Linking your CGM with a smart device or insulin pump? Download the CGM device summary to find out which devices are compatible. How many products you can access There are limits on how many CGM products you can access each year. These limits are based on: the lifespan of each CGM device, as advised by the manufacturer how many CGM products you have accessed in the past 12 months from the present date. We recommend ordering one month’s supply at a time, as CGM products have a short shelf life. Re-order your sensor around 14 days before you run out, or when you start using your second-last sensor. This helps make sure you always have supplies. We understand that some people may need more products to manage their diabetes. Call the NDSS Helpline on 1800 637 700 for more information.
