NSW

Get healthy is designed for people living in NSW who are at risk of developing type 2 diabetes. It is a free service. You receive up to 6 or more telephone calls from a personal health coach, who can motivate and support you to set your own healthy lifestyle goals. Qld

My health for life is a free, healthy lifestyle program for Queenslanders who are at high risk of chronic disease, including type 2 diabetes.

Designed to support positive behaviour change, the program can help women who have had gestational diabetes to plan and action small lifestyle changes that have long term health benefits. The program involves six sessions overs six months and is delivered by qualified health professionals. Participants have the choice of group sessions or phone coaching options.

SA

The Better Health Coaching Service is a free, confidential, telephone-based program designed to help South Australians be active, eat well and reduce the risk of chronic disease. The program links participants with a qualified health professional and provides up to 10 free coaching calls over six months.  

The Better Health Coaching Service is free to eligible South Australians aged between 18 and 74 years, with a BMI equal to or more than 27 and seeking support to build and maintain healthy lifestyle habits. To find out if you’re eligible, talk to your GP or health professional about options that are best for you. Tas

The COACH Program® provided by Diabetes Tasmania is a free telephone coaching prevention service for Tasmanians at risk of or diagnosed with type 2 diabetes or heart disease. It provides people with the opportunity to work with a coach (health professional) to understand, manage and improve their health, particularly around the risk factors associated with diabetes and its complications.

Vic

The Life! program is a free Victorian healthy lifestyle program that helps you improve your eating habits, physical activity and stress management. Run by expert health professionals, the program is delivered as a group course or telephone health coaching service and shows you how to reduce your risk of developing type 2 diabetes and cardiovascular disease. If you were diagnosed with gestational diabetes during pregnancy, you are automatically eligible to participate.

Changing your lifestyle isn’t easy, especially on your own. The Life! program gives participants the motivation and support needed to make and maintain positive changes and to live a healthier and more active lifestyle. This approach has been shown to be more effective than taking medication and has a long-term positive effect on your health.

Since 2007, over 75,000 Victorians have learnt more about the steps they can take to live a healthy life. Funded by the Victorian Government and coordinated by Diabetes Victoria, Life! is the largest prevention program of its kind in Australia.
