NDSS feedback form

This form can be used to provide feedback about the NDSS by people with diabetes (and carers) who are registered with the NDSS. To use the form you must have a NDSS registration number.

Any feedback given is used to ensure that the NDSS continues to provide the highest possible level of service and support.

Should you require any urgent medical attention please call 000 or visit the emergency department at your nearest hospital.
