Participate in shaping the future of the NDSS: Take part in the Annual Health Provider Survey

At Diabetes Australia, we are dedicated to advancing our services. To support this mission, we invite health professionals to contribute to the enhancement of National Diabetes Services Scheme (NDSS) through our annual survey. This survey serves a dual purpose: gaining insights into supporting people living with diabetes and evaluating the effectiveness of the NDSS.

Take our brief 2-8 min survey to shape the future of diabetes care:

Conducted independently by the University of Technology Sydney (UTS), this survey adheres to all privacy regulations. All responses will remain anonymous, and no personally identifiable information will be shared with Diabetes Australia. For any inquiries about the survey, please reach out to UTS at [email protected] or call 02 9514 8021.

Diabetes Australia follows the strictest confidentiality guidelines. We would like to assure you that your personal information has been protected. You can view our privacy policy here.

We extend our gratitude for the time you invest in helping us and fellow individuals living with diabetes. Together, let’s make a positive impact on supporting people living with diabetes and improving the effectiveness of the NDSS.
