As an Australian Government initiative administered by Diabetes Australia, the NDSS helps you manage your health with free education, tools, and services. If eligible, you can sign up easily by asking your health professional at your next appointment to fill out this form. How do I register? Step 1: Book an appointment We’ve made registration easy, so you can focus on living well, not paperwork. Just book an appointment with your health professional and ask them to register you with the NDSS. That’s it, they’ll handle the rest. Health professionals who can register you include: Aboriginal Health Practitioners Credentialled Diabetes Educators Endocrinologists General Practitioners (GPs) Nurse Practitioners Practice Nurses If you don’t currently have a regular health professional, you can use the NDSS health professional locator, shown below, to find one nearby. Telehealth services are also available, and in some cases, your local pharmacist may be able to assist. Find a health professional Step 2: Access support while you wait While waiting for your physical NDSS registration card, you can still access NDSS products, services, and support. Simply present one of the following at any NDSS Access Point: Your Medicare card Your NDSS registration number (if available) If you experience any issues accessing subsidised products, contact the NDSS Helpline on 1800 637 700 for assistance. Am I eligible? You may be eligible to register if you: Have been diagnosed with any type of diabetes by your doctor OR Have another eligible condition that needs regular monitoring of blood glucose levels. AND Live in Australia and hold (or are eligible to hold) a Medicare card or Department of Veterans’ Affairs cardOR Are an area resident of a country with which Australia has a Reciprocal Health Care Agreement (visit the Service Australia website for details). People with type 1 diabetes, type 2 diabetes, gestational diabetes, or ‘other’ diabetes may apply to be registered with the NDSS. Who is not eligible?
