Help your patients stay on top of their diabetes

The National Diabetes Services Scheme (NDSS) can help your patients stay on top of their diabetes. Registration is free, and it will give them access to a great range of services including workshops, programs, information, resources, peer support and subsidised products.

Step 1

Access and complete the NDSS Registration form.

Add the patient’s email address or mobile number for the timely return of their NDSS Registration number.

Step 2

Print it out, sign to certify registration and ask your patient to sign.

Step 3

You or your patient can send the completed and signed form to NDSS:

Post: GPO Box 9824 in your capital city.

NDSS Access Point: Many pharmacies are NDSS Access Points. Your patient can ask if their pharmacy will accept this form.
